Körperliche Untersuchung: German Physical Exam Commands
You've finished the history; now it's time for the physical exam (körperliche Untersuchung). This is the most hands-on part of both the FSP and daily clinical work: you guide the patient with clear, precise commands and then describe your findings in medical language. Two registers run at once — everyday German commands to the patient, Fachsprache findings for the chart and your colleagues. Below I've collected the most frequently used phrases system by system; make them second nature.
1. General opening and preparation commands
Before you start, inform the patient and ask permission — this is both courtesy and points on the exam:

2. The four basic techniques: Inspektion · Palpation · Perkussion · Auskultation
The classic exam sequence is these four steps. Knowing their names and what they mean lets you give the right command and write up the finding correctly:

Practical tip: for the abdominal exam the order changes — first Inspektion, then Auskultation (so palpation doesn't disturb the bowel sounds), and only then Palpation and Perkussion. Knowing this order is a bonus point in the FSP.
3. Herz and Lunge (heart and lungs)
The commands you'll use most in the thoracic exam — give the breathing commands slowly and clearly in particular:

4. Abdomen (the abdomen)
In the abdominal exam, putting the patient at ease and getting pain feedback is critical:

5. Neurological examination
Typical instructions for reflexes, strength and coordination:

6. Extremitäten (limbs) and movement

7. Describing findings in Fachsprache
The second half of the FSP: putting what you saw on examination into medical language in writing or speech. The most common "normal/pathological" pairs:

8. Patient guidance and courtesy phrases
- "Ich erkläre Ihnen, was ich als Nächstes mache." — Let me explain what I'll do next.
- "Das war's mit der Untersuchung, vielen Dank für Ihre Geduld." — That's it for the examination, thank you for your patience.
- "Sie können sich jetzt wieder anziehen." — You can get dressed again now.
- "Bitte sagen Sie mir, wenn Ihnen schwindelig wird." — Please tell me if you feel dizzy.
- "Ich fasse Sie jetzt am Bauch an, ist das okay?" — I'm going to touch your abdomen now, is that okay?
9. Common mistakes
- Using medical terms with the patient. Not "Ich palpiere jetzt Ihr Abdomen" but "Ich taste jetzt Ihren Bauch ab". Commands in everyday language, findings in Fachsprache.
- Giving vague commands. "Atmen Sie" isn't enough; say it clearly and step by step, like "Atmen Sie tief durch den Mund ein und aus".
- Skipping permission/information. Saying "ist das in Ordnung?" before you touch earns points; skipping it loses them.
- Not asking for pain feedback. During palpation, don't forget to say "Sagen Sie, wenn es wehtut".
- Building commands with English/Turkish logic out of nerves. Memorise the German phrase as a whole, don't translate it word for word.
A striking truth: examiners most often measure not your technique but your communication. The candidate who guides the patient kindly and explains every step in advance always starts ahead of the one who silently grabs hold of the patient.
